This Web site was created to assist Florida stakeholders in the development and implementation of a strategic plan to improve health care transition outcomes for youth and young adults with disabilities, chronic health conditions or other special health care needs.
Increasing awareness of the difficulties youth with disabilities encounter when they transition from pediatric to adult health care prompted passage of Senate Bill 988/House Bill 793 during the 2008 Florida legislative session. Effective July 1, 2008, SB 988/HB 793 called for the establishment of a statewide Task Force to assess the need for health care transition services, develop strategies to ensure successful transition from pediatric to adult health care systems, and identify existing and potential funding sources.
